This is the first step for access to the Community Care Program and a broad range of services
to seniors. A FREE preliminary assessment will determine which services the client is eligible to
receive.

MDH is a 113-bed healthcare facility providing a large spectrum of advanced medical treatments and personalized health services. MDH is accredited by The Joint Commission and is a member of Voluntary Hospitals of America (VHA). MDH provides a wide range of special programs for seniors, including:
- Adult Day Health Services: is a day program for adults 60+ who are in need of health-related services or social activities. It is also a regional CST Emergency Response System provider and installer for Illinois. Open M-F 8:30-4:30 pm. 309-836-1656

- Emergency Shelter Program
- Food Pantry
- Home Weatherization Assistance
- Home Maintenance Program for the elderly and disabled, provides minor modifications to owner-occupied homes to enable individuals to remain in their homes.
- Low Income Home Energy Assistance (LIHEAP)
- Domestic Violence and Sexual Assault services, free and confidential
Food Pantry Hours:
- Monday through Friday, 8:00 am-4:00 pm
- Food available once every 30 days to income eligible house holds.

YMCA Senior Center

This is the focal point for Senior services funded by the Western Illinois Area Agency on Aging.
- Eligibility for services: age 60 and over or disabled. All services and activities, except meals, are free.
- Nutritional Services: Dine-in and Home Delivered Meals at five sites-Macomb, Bushnell, Colchester, Blandinsville, and Industry
- Information and Assistance (I & A) and Outreach Services: Assistance with applications for Illinois CaresRx, Circuit Breaker, Medicare Part D, and Property Tax Freeze, S.H.I.P. for Medicare problems. Makes referrals to other agencies.
Activities available for seniors include:
- Bridge, Pitch, Scrabble, Pool (Billiards)
- Dancing (Square and Line)
- Sit 'N Tone-Exercise for mobility-limited seniors
